1) Securities Transfer Forms:
Canadian Western Bank Declaration of Ownership - To be completed by the transferee and submitted with all transfers of shares of Canadian Western Bank.
Corporate Resolution - A sample form of the resolution that must accompany all requests to transfer securities from a corporation to another party. An original or certified copy is required and it must be dated within six months. Please note that a resolution is not required for corporate transfers where the signature(s) on the Stock Power of Attorney have been Signature Medallion Guaranteed.
Declaration of Transmission - The required sample form that must accompany all non-probated estate transfers and probated estate transfers with a Canadian Schedule I bank signature guarantee on the Stock Power of Attorney rather than a Medallion guarantee.
Irrevocable Stock Power of Attorney - A sample form required for transferring securities from the registered owner (the transferor) to a specified transferee. An original is required. The signature(s) of the registered owner(s) or their authorized representative must be signature guaranteed by a Canadian Schedule I Chartered Bank or a member of recognized Medallion Signature Guarantee Program (STAMP, SEMP or MSP). Please include the complete address of the transferee and their Social Insurance Number for tax reporting purposes.
Sole Signing Officer Resolution - The required sample form of resolution for companies with only one signing officer/director and for sole proprietorships. An original is required.
2) Direct Deposit Request Forms:
The following issuers that make regular dividend or interest disbursements to their securityholders, offer the option of deposit of disbursements directly into the holder's Canadian bank account. If you, as a securityholder of one of these issuers, would like to request this service instead of receiving cheques by mail, please complete the applicable form and return it to our office at the address shown on the form.

3) Address Change Request Form - To be completed by a registered securityholder wishing to update their address of record on the books of a company whose records are maintained by Valiant Trust.
4) Optional Cash Plan (OCP) Declaration Form - To be completed by any securityholder intending to participate in an Optional Cash Plan offered under a Dividend Reinvestment Plan managed by Valiant Trust.
5) Politically Exposed Foreign Person (PEFP) Declaration (Wire Transfer Beneficiary) - To be completed by the beneficiary of any international wire transfer being sent by Valiant Trust.
